Terms and Conditions of Service

  • As part of providing a therapeutic service to you, Brooke Lumsdaine Therapy will need to collect and record personal information that is relevant to your current situation. This information is necessary to inform part of the assessment and therapy offered to you. I will not seek personal information that does not pertain to our therapeutic work together. If I consider your needs are beyond the scope of my competence and training, I reserve the right to end our agreement. This will be discussed in the session, and onward recommendations may be provided.

    Professional Ethics

    Having trained in Australia, I adhere to both the Australian Association of Social Workers (AASW) Code of Ethics (2020) and the British Association of Social Workers (BASW) Code of Ethics (2021) where I currently practice. Overarching Social Work values that are integral to the profession and key to my practice are respect for persons, social justice and professional integrity.

    Purpose of collecting and storing information

    The information gathered informs part of the assessment and service provided to you. It is only seen by your therapist, unless in cases of an urgent nature - such as an emergency, or regarding risk or safeguarding concerns. Notes may be taken during or after the session, which will be kept in accordance with the Data Protection Act (1998). As such, records may be kept for up to seven years, unless following a final session you indicate that you would like your personal records be immediately deleted. In this case I will terminate your file as per your request.

    Access to Client Information

    At any stage, you the client are entitled to access your personal information kept on file, unless the relevant legislation provides otherwise. Please discuss with your therapist appropriate forms of access, if required.

    Confidentiality

    All information gathered by the therapist during the therapeutic service is confidential and will remain secure except where:

    1. Failure to disclose the information would place you or another person at serious and/or imminent risk.

    2. There may be some legal limits to confidentiality;

    a. Where the client, a child, or a vulnerable adult is at risk.

    b. Where people are judged to be at risk through acts of terrorism.

    c. Where there is a police investigation.

    d. In a Court of Law.

    3. Your prior consent has been obtained to;

    a. Provide a written report to another professional agency. (e.g, a GP or Referral)

    b. Discuss the information with another person (e.g, a parent, employer, healthcare provider. Or if disclosure is otherwise required or authorised by law).

    4. All information provided by client is kept in accordance with the GDPR legislation (May 2018). This means that your information will not be shared with third parties, except for those outlined in points 1, 2 or 3.

    5. As part of my code(s) of practice I am required to carry out continuing professional development, and to engage in regular on-going clinical supervision. This is to ensure an ethical and professional service to clients. I may discuss your case in supervision but would not use any identifying details.

    Safety

    To support the idea of therapy as a productive and affirming process, I will create a safe and confidential environment for you. I also invite you to take personal responsibility during our work together. In regards to safety you agree:

    • not to harm yourself or another.

    • not to attend sessions under the influence of alcohol or non-prescribed substances.

    • not to record our sessions. I do not give consent to be recorded on any platform.

    • during online sessions, you will participate from an appropriate space to ensure our therapy time is not interrupted and remains confidential. No unidentified people will be present.

    • If you are currently seeing a psychiatrist, or if you are in regular contact with your GP regarding a mental health issue that you are also seeking therapy for, I reserve the right to inform them of my contact with you at any stage of therapy, as they carry clinical responsibility for your care.

    • I reserve the right to terminate a session if it is unsafe, or you have not honoured these terms.

    Fees

    All fees are payable in advance, via the booking portal on my website.

    The cost and length of a therapy session is outlined on my website and is subject to change, according to annual reviews. I will give you advance notice of any change to service fees. Fees and length of sessions are structured to optimise the particular therapeutic service being offered and to compensate clinical and administrative time on the therapists part.

    Cancellation Policy:

    Your confirmed session is reserved for you and so booked out in advance in my calendar. I respect your time and my own. If you need to cancel or reschedule I require 48 hour notice, otherwise the fee for this session is non-refundable. Missed sessions without notice are also non-refundable.

    If I need to cancel your session, I will always give you as much notice I can. We will reschedule your appointment or you can request a full refund.

  • 1) I agree to take personal responsibility for my experience, before, during and after this Family Constellation Session. Family Constellations can be emotionally confronting and may cause me to experience distress or physical responses that may be unexpected and/or unpleasant. This method may be a challenging and/or unsuitable modality for people with clinical mental health conditions (such as schizophrenia and/or bi-polar disorder). Please disclose any mental health conditions to Brooke, prior to the appointment.

    2) I understand that this session should not be used as a substitute for therapy or medical treatment.

    3) I understand that there is a risk of accident, injury, and emotional distress. I agree to assume this risk, including but not limited to the types of responses described.

    4) I am a willing, healthy and consenting adult participating in this modality. I agree not to participate under the influence of substances. I confirm that I do not suffer from any mental or physical impairment and have not been diagnosed at any point with a disorder, condition or injury, either physical or mental, that might make it inadvisable for me to assume such risks.

    5) I understand that there are no guaranteed outcomes offered.

    6) I understand that I may leave this session at any time for any reason. I understand that I will not be refunded or offered a replacement session.

    7) I willingly agree to the preceding statements and to hold harmless from all liability, the facilitator, Brooke Lumsdaine.

Please disclose any mental health conditions to Brooke, prior to the workshop.

    2) I understand that this workshop should not be used as a substitute for therapy or medical treatment.

    3) I understand that there is a risk of accident, injury, and emotional distress. I agree to assume this risk, including but not limited to the types of responses described.

    4) I am a willing, healthy and consenting adult participating in this workshop. I agree not to participate under the influence of substances. I confirm that I do not suffer from any mental or physical impairment and have not been diagnosed at any point with a disorder, condition or injury, either physical or mental, that might make it inadvisable for me to assume such risks.

    6) I understand that I may leave this workshop at any time for any reason. I understand that I will not be refunded or offered a replacement workshop.

    7) I willingly agree to the preceding statements and to hold harmless from all liability, the facilitator, the organisers and all participants and observers attending this workshop.

    Cancellation Policy:

    Your confirmed place in the workshop is reserved for you. If you need to cancel or reschedule I require 48 hours notice for a full refund (minus a £10 administration fee). If you cancel less than 48 hours before a scheduled workshop, you will receive a 50% refund. Non-attendance without notice is non-refundable.

    If a workshop is cancelled, you can either attend the next available workshop date, or, request a full refund.

  • 1)        I agree to take personal responsibility before, during and after this Family Constellation Workshop. Family Constellations can be emotionally confronting. Especially within a group of strangers as representatives. This method may be a challenging and/or unsuitable therapy for people with clinical mental health conditions (such as schizophrenia and/or bi-polar disorder). Please disclose any mental health conditions to Brooke, prior to your Family Constellation Workshop.

    2)        I am a willing, healthy and consenting adult participating in this modality. I agree not to participate under the influence of substances.

    3)        I understand that there are no guaranteed outcomes offered.

    4)        I agree to follow up contact via email or phone, by Brooke, in the weeks following this workshop.

    Cancellation Policy:

    What happens if I need to cancel or can’t be there on the day we confirmed?

    Unfortunately, once we have confirmed and paid for the venue we are going to work from, there is no refund. Due to the administrative time, the personal time volunteered by participants and the cost of a private venue, the workshop will go ahead in your absence. As a gesture of goodwill, I will endeavour to contact the other clients who have registered their interest in a personal constellation workshop, to see if they are available and willing to have their own constellation on this day and time. If someone else does confirm, I will refund you your client fee cost of £250.00. However, this is no guarantee.

     What happens if you can’t be there on the day we confirmed?

     If in the unlikely event that I need to cancel, I incur the cost of the venue booked. I will be in touch to reschedule your workshop. If you decide at this time that you would not like to reschedule, you are entitled to request a full refund of the client fee, rather than reschedule.

    4) I agree to maintain confidentiality regarding any information shared by other participants during this workshop.

    5) Participation as a representative is Free, however I commit to attending the agreed date that I have confirmed, to ensure the constellation can go ahead. I understand that the facilitator reserves the right to offer my place to another willing representative if I do not attend a workshop I had previously agreed to attend.

  • Social Work Supervision Service

    As part of providing a Social Work Supervision service to you, Brooke Lumsdaine Therapy will need to collect and record personal information that is relevant to your current situation (i.e identifying details, your current workplace and role). This information is necessary to gain context to your Social Work and/or team’s role, and the Supervision service offered to you and/or your team. I will not seek personal information that does not pertain to our work together. If I consider your needs are beyond the scope of my competence and training, I reserve the right to end our agreement. This will be discussed in the session, and onward recommendations may be provided.

    Following agreement to our terms of service, you will be sent a Social Work Supervision Contract to complete and return prior to our first session together. The supervision contract is designed to be a collaborative, working document, agreeable by both the supervisor and supervisee. This is to ensure that supervision remains tailored to the supervisee’s goals, learning and development.

Notes may be taken during or after the session, which will be kept in accordance with the Data Protection Act (1998). As such, records may be kept up to 7 years, unless following a final session you indicate that you would like your personal records be immediately deleted. In this case I will terminate your file as per your request. 

    Access to Information

    At any stage, you the supervisee are entitled to access your personal information kept on file, unless the relevant legislation provides otherwise. Notes recorded in Supervision will be stored securely by Brooke Lumsdaine Therapy, and, a copy of our session will be sent to you to sign and store also.

    All information gathered by your Supervisor during supervision is confidential and will remain secure except where:


    1. Failure to disclose the information would place you or another person at serious and/or imminent risk.

    All fees are payable in advance, via the booking portal on my website. If you cannot find a time that you are available through the online booking portal, please email me with your alternative preferred times. Once / If a suitable time is agreed upon, I will send you a direct booking link. 

    The cost and length of an Individual Social Work Supervision session is £120 for 50 minutes. The cost and length for Group Social Work & Systemic Supervision correlates to the particular package you are choosing (a one-off, bundle, or ongoing) outlined on my website. Cost is £170 per hour (at 2 hours per session) + travel time & expenses. The fees are subject to change, according to annual reviews. I will give you advance notice of any change to service fees. Fees and length of sessions are structured to optimise supervision and to compensate clinical and administrative time on the supervisors part.
